Who was on the shift when you fell? Based on your posts here, it was either Monday the 25th of January or Tuesday the 26th. You said that no one saw you, but if you said anything to anyone, that would still be a witness after the fact.
Damn. It is why I said it needed to be documented, including making certain there was a record of the event. Without that, without a journal of your own, the only record is here - and this isn't exactly a place that you can use as a reference.
You had to spend time at home due to back pain. Shouldn't that have been on record when you missed work? February 24th as well as this Sunday.
Use the free clinic list. Call one that looks promising. Even if it isn't the right one, they may know which of the others would be. You don't need health insurance for free clinics. That's why they exist.

No union and you're in Tennessee, which is a deep red state. I don't know if there is an ombudsman around. Sometimes there are organizations that look into those things. Some newspapers and television stations have those, but newspapers have been closing and those that haven't are often bought out by chains looking to cut them to the bone. Just brainstorming.
If it's a large franchise then there will be others affected. If it's small and regional, less chance of that but there could still be something. Any labor organizing force keeps a record of things like that. I'm certain that there are NGOs (non-governmental organizations) that try to track it even if the state government doesn't have a keen OSHA of its own. I don't know the reputation in TN.
If they knew what happened and failed to file, that is a thing. Know your rights under Tennessee law. Even if you can't get workman's comp directly, you might manage something. Getting checked out as soon as possible is important, especially if you can be helped. Getting any problem fixed it a higher priority than anything else.

That is good as long as you're careful. Pain management can be tricky. Better that than medication.
Have you been able to schedule an appointment at one of the free clinics or at least find one that might be able to help?
